Local SEO vs GEO for dental clinics: what's the difference and do you need both?

They share a foundation, not a finish line
Local SEO and GEO (Generative Engine Optimization) both start from the same place: an accurate, complete Google Business Profile and consistent business information across the web. Where they diverge is what they are optimising for. Local SEO is aimed at ranking in Google's traditional map pack and organic local results. GEO is aimed at being named directly inside an AI-generated answer — ChatGPT, Perplexity, Google's AI Overviews, Gemini — without the patient ever seeing a list of options to choose from.
The gap between these two outcomes is large and measurable. SOCi's 2026 research found businesses appear in Google's local 3-pack 35.9% of the time, but the same businesses are recommended by ChatGPT only 1.2% of the time and Perplexity 7.4% of the time. Strong local SEO does not automatically produce strong GEO. It is necessary, but it is not sufficient.
Where local SEO stops and GEO picks up
Local SEO's core toolkit — Google Business Profile optimisation, citations, on-page local keywords, map pack presence — genuinely does feed the AI Overview and Gemini layer, since those platforms are grounded in Google Maps data. That overlap is real, which is why getting your Google Business Profile right is not wasted effort even from a pure GEO perspective.
But ChatGPT and Perplexity draw from a much wider, less controlled mix of sources: your website's actual content, review platforms, directories, professional profiles, and editorial mentions. GEO's additional toolkit — structured schema markup, rigorous cross-platform NAP consistency, genuinely answer-first content, and a real publishing cadence — is what extends visibility beyond the map pack into these less predictable, harder-to-influence surfaces.
Why a clinic needs both, not one or the other
Patients are not choosing one search behaviour and sticking to it. The same person who checks Google Maps for hours and distance might separately ask ChatGPT for a recommendation from a friend's phone, or ask Perplexity to compare treatment costs before calling anyone. A clinic that has only invested in local SEO is fully visible for the first behaviour and functionally invisible for the second and third — not because it did anything wrong, but because those are different systems evaluating different signals.
The practical order matters, though. Local SEO fundamentals — an accurate, complete Google Business Profile above everything else — are the correct starting point, because they are lower effort, more within a clinic's direct control, and partially double as GEO groundwork. GEO-specific work — schema, structured content, publishing cadence — is the next layer once that foundation is solid, not a replacement for it.
