AI search visibility for dental clinics: the complete guide
Ranking well in Google no longer means an AI names you. This is the definitive breakdown of how ChatGPT, Perplexity and Google's AI actually pick a dentist — and the six factors that decide whether yours is one of them.

The shift already happened. Most clinics have not noticed.
We have already covered why this matters and what it costs a clinic in missed patients. This piece is the deeper reference: exactly how these systems decide, platform by platform, and exactly what to do about it — in order, starting with the highest-leverage fix.
Here is the harder truth first. Ranking well in Google's local pack does not mean an AI will name you. SOCi's 2026 Local Visibility Index, which analysed AI recommendation behaviour across nearly 350,000 business locations, found that brands appear in Google's local 3-pack 35.9% of the time — but the same brands are recommended by ChatGPT only 1.2% of the time, and by Perplexity 7.4% of the time. Search Engine Land's independent coverage of the same report put it plainly: AI visibility is three to thirty times harder to earn than traditional local search visibility. Most clinics are optimising for the game that got easier and ignoring the one that got harder.
How an AI actually decides which dentist to name
Neither ChatGPT nor Perplexity queries a single database when someone asks for a dentist recommendation. There is no dental-clinic index they look up. Instead, the model synthesises an answer in real time from whatever public information it can find and trust: your website, directory listings, review platforms, and any editorial or third-party mentions across the open web — then names one to three businesses it can describe with confidence.
“With confidence” is the operative phrase. These systems are built to avoid recommending something they cannot verify, because a wrong recommendation costs them more credibility than a missed one. A clinic that ranks well for a keyword can still lose the recommendation if its information is inconsistent across the web — one address on your website, a different one on your Google Business Profile, a third on a directory that has not been updated since 2022. The AI does not resolve the contradiction in your favour. It simply loses confidence and picks a competitor whose data agrees with itself.
This is why SOCi's report attributes the ChatGPT and Perplexity accuracy gap — both platforms show business contact information that is only about 68% accurate, versus 100% on Gemini, which reads directly from Google Maps — not to platform failure but to inconsistent source data. The fix is not clever prompting or a paid placement. It is making the boring facts about your clinic agree with themselves everywhere they appear.
Four platforms, four different games
Optimising for one AI engine does not carry over to the others. Research on citation overlap found that 71% of AI citations are platform-specific — a source cited by ChatGPT is very often not the same source cited by Perplexity for the identical question. Here is how the four that matter for a dental clinic actually differ:
Google AI Overviews and Gemini are grounded directly in Google Maps and Google Business Profile data, which is why Gemini's business information is 100% accurate against SOCi's benchmark and recommends businesses roughly 11% of the time — far above ChatGPT or Perplexity. If your Google Business Profile is complete and correct, you are already most of the way to visibility here.
ChatGPT leans on general reasoning and pre-trained trust in sources rather than live web synthesis in every case, which is part of why it recommends only 1.2% of locations in SOCi's study — the most selective of the four platforms measured.
Perplexity behaves more like a research assistant, pulling sourced answers from the live web and weighing five signals when choosing what to cite: freshness, structured Q&A formatting, authority within its source pool, direct query-to-heading matches, and citation density. Perplexity weights recency unusually heavily — a page published two weeks ago will out-cite a page published eight months ago in more than 70% of direct comparisons, all else equal. This is precisely why we publish daily rather than monthly.
The practical conclusion: you cannot pick one platform and call AI visibility solved. You need your Google Business Profile complete (for Google AI Overviews and Gemini), your on-site facts consistent and your entity clearly defined (for ChatGPT), and your content structured and fresh (for Perplexity).
When Google actually shows an AI Overview for a dental search
Not every search triggers an AI Overview, and the rate varies enormously by query type — which matters because it tells you where to focus. Search Engine Journal's 2026 analysis found direct, high-intent local service searches like “dentist near me” trigger an AI Overview relatively rarely — roughly 7% to 20% of the time — because Google still tends to serve the traditional map pack for that kind of immediate, transactional intent.
Informational and comparison queries are a different story. Searches like “how much do dental implants cost” or “best dentist in Visakhapatnam for root canal” trigger AI Overviews far more often — commonly cited in the 76% to 97% range depending on how specific and comparison-oriented the question is.
The implication for a clinic: the content most likely to earn an AI citation is not your homepage or your “book now” button. It is the page that actually answers the question a patient is asking before they have decided which clinic to call — cost breakdowns, treatment explanations, comparison content. That is exactly the gap this Journal exists to fill.
The six factors that decide whether your clinic gets picked
Strip out the platform-specific detail and six factors keep surfacing across every study of AI local recommendation behaviour:
- Data accuracy — your name, address, phone number and hours must be identical everywhere they appear. Not similar. Identical.
- Review quality, not volume — AI platforms use reviews as a confidence threshold, not a ranking gradient. Locations recommended by ChatGPT average 4.3 stars in SOCi's data. A dozen detailed, specific reviews beat fifty generic five-star ratings, because the model reads review content, not just the star count.
- Structured content — clear headings, direct answers near the top of the page, and genuine FAQ content the model can extract cleanly.
- Cross-platform consistency — your Google Business Profile, website, Bing Places, Apple Maps, Facebook and any directory listing all need to agree.
- Schema markup — machine-readable LocalBusiness and MedicalOrganization structured data that states unambiguously what you are, where you are, and what you treat.
- Freshness — particularly for Perplexity, but relevant everywhere: a site that visibly publishes is trusted more than one that has not changed since launch.
None of these six require a redesign. They require the boring, unglamorous work of making the facts agree with themselves and publishing consistently — which is exactly why so few clinics have done it yet.
Check where you stand before you spend anything
You can check most of this yourself in twenty minutes. Ask ChatGPT, Gemini and Perplexity separately “who is the best dentist in [your city] for [your top treatment]” and note whether you appear, and what is said about you if you do — the wording often reveals exactly which source the model trusted. Then pull up your Google Business Profile, your website's contact page, and any directory listing you can find, and compare the address, phone number and hours character by character. Contradictions hide in places you would not expect — a directory nobody has logged into since 2022 is a common one.
If you would rather have this done properly rather than manually, a free clinic audit covers exactly this: your Google Business Profile field by field, what the major AI engines currently say about you, and where your booking path loses a ready patient. No sales call required to get it.
Building visibility, in order
Sequence matters here, because some fixes unlock the value of the ones after them. Start with your Google Business Profile — it is the highest-leverage single fix, since it feeds Gemini and Google AI Overviews directly and is the profile most consumer-facing directories reference. We cover this field by field separately, because most clinics fill in four fields and leave the rest blank.
Next, audit and fix NAP consistency across every platform where your clinic appears — website, GBP, Bing Places, Apple Maps, Facebook, any dental-specific directory. Then add proper schema markup to your website so machines have a structured, unambiguous statement of what you are and where you are, rather than having to infer it from prose. Then build genuine FAQ content around the specific questions patients actually search — cost, timing, insurance, first-visit process — rather than generic service descriptions. Finally, establish a publishing cadence. One well-structured page a week beats ten pages published once and never touched again, particularly for Perplexity's recency weighting.
What actually kills AI visibility
The mistakes are consistent across every clinic we audit. Inconsistent NAP data is the most common and the most damaging, because it directly undermines the entity confidence every platform is built around. Thin or generic reviews are second — fifty reviews that all say “great service” carry less weight than a dozen that mention specific treatments and outcomes. No schema markup is third: without it, an AI is inferring your specialty and location from unstructured text instead of reading it directly. And chasing keyword rankings while ignoring entity clarity is the fourth — a page can rank well in classic search and still never earn an AI citation, because ranking and being trusted enough to recommend are not the same signal.
